Behavioral Activation
Behavioral activation (BA) is a talk therapy for depression that works from the outside in: you plan small activities that bring pleasure or matter to you, do them before you feel like it, and rate your mood before and after. A Cochrane review of 53 studies found it worked about as well as cognitive behavioral therapy in the short term [1].
Your week of activities
Plan a few small things, rate your mood from 0 (worst) to 10 (best) before and after, and tick what you did.
- Mon · Small task Wash the dishes from one meal Done. Mood 3 before, 4 after
- Mon · Pleasure Listen to one favorite song all the way through Done. Mood 3 before, 5 after
- Tue · Meaning Call or message one person you care about Done. Mood 2 before, 4 after
- Wed · Pleasure Sit outside with a coffee for 10 minutes Planned
- Thu · Small task Open the mail and sort it into two piles Planned
- Fri · Meaning Cook one simple meal for yourself or someone Planned
- Sat · Pleasure Take a short walk around the block Planned
Done 3 of 7. Mood change after an activity: +1.7 on average (pleasure +2, meaning +2, small task +1).

How behavioral activation works
You keep telling yourself you will call a friend back "later, when I have the energy." Low mood makes you do less, and doing less leaves fewer moments of pleasure or contact with people, so the mood drops further.
Behavioral activation breaks that loop by changing what you do. You don't wait to feel better before you do things. You schedule small, concrete activities and let the mood follow, or not, and you write down what happens. Over a few weeks, your own ratings show which activities raise your mood and which lower it.
NICE, the UK body that writes treatment guidelines, describes it as focusing on the link between a person's activities and their mood, without targeting thoughts and feelings directly [2].
How to do behavioral activation: 5 steps
At first, give each of the first three steps a few days.
- Notice what you do now. For two or three days, jot down what you did each part of the day and your mood from 0 to 10. No changes yet.
- Make a list of activities. Write down things that used to give you pleasure, things that matter to you (people, care, values), and chores you have been avoiding. The planner sorts them as pleasure, meaning and small task.
- Cut each one down. "Clean the house" becomes "wash the dishes from one meal." If you would not do it on a bad day, it is still too big.
- Put 1 to 3 of them on specific days. A day and, if you can, a time. Mix the kinds, so a week is not only chores.
- Do it, rate it, review it. Rate your mood just before and just after. At the end of the week, keep what helped, make the rest smaller, and plan the next week.
Do the activity because it is on the plan, even if you don't feel ready.
Behavioral activation examples
A good week mixes all three kinds of activity, and chores alone tend to feel like a punishment.
| Kind | Examples | The smallest version |
|---|---|---|
| Pleasure | music, a show, a walk, a warm shower, a hobby you dropped | one song all the way through |
| Meaning | contact with people, caring for a pet or plant, helping someone, faith or community | a two-line message to one person |
| Small task | dishes, mail, laundry, one email, a bill | open the mail and sort it into two piles |
Pick activities that fit your life. A parent might choose reading one picture book with their child; someone who loved the gym might start with a 10-minute walk. Add one to the planner now, for tomorrow.
Does behavioral activation work? What trials found
In a 2007 meta-analysis of 16 studies with 780 people, the effect of activity scheduling against control conditions was large, 0.87, and in the 10 studies that compared it with cognitive therapy, the difference was 0.02 [3].
| Study | What it found | Caveat |
|---|---|---|
| Ekers 2014, 26 trials [4] | Better than control conditions (SMD −0.74); in 4 small trials, slightly better than medication (SMD −0.42), with a confidence interval that reaches zero | Study quality was low in most trials, and follow-up was short |
| COBRA 2016, 440 adults [5] | After 12 months, BA from junior mental health workers did as well as CBT from therapists: both groups averaged 8.4 on the PHQ-9 | One country (UK); 79% of the BA group could be assessed at the end |
| Cochrane 2020, 53 studies [1] | Similar to CBT (risk ratio 0.99) and more effective than usual care (1.40) in the short term | The advantage over usual care disappeared in worst-case analyses; certainty was moderate |
| Alber 2023, 12 online trials [6] | Internet-based BA lowered depression compared with inactive controls (SMD −0.49) | No effect was found at 6 months; all studies had some risk of bias |
The evidence is about a treatment, usually with a therapist or a guided program. Our planner has not been tested, and the trials say little about doing BA completely alone.
Is behavioral activation CBT?
Partly. Behavioral activation began as one component of cognitive behavioral therapy for depression.
In 1996, Jacobson and colleagues gave 150 people with major depression the full cognitive therapy package, its behavioral activation part alone, or activation plus work on automatic thoughts. The full package did no better, at the end of treatment or 6 months later [7]. A 2006 trial with 241 adults then compared BA with cognitive therapy and antidepressants: among the more severely depressed patients, BA did as well as medication, and both did better than cognitive therapy [8].
The difference is focus: CBT also works on the content of thoughts, while BA mostly leaves thoughts alone and changes what you do. If looping thoughts are the bigger problem for you, our page on how to stop overthinking covers a method aimed at them.
Doing it on your own or with a therapist
NICE lists behavioral activation in three forms for less severe depression: guided self-help with a trained practitioner over 6 to 8 sessions, a group over 8 sessions, and individual therapy over 12 to 16 sessions [2].
- If your low mood is mild and you are still mostly getting through the day, a self-help plan like the one above is a reasonable start. Tell someone you trust that you are trying it.
- If you have tried for 2 or 3 weeks and keep stalling, ask your doctor about guided self-help or a therapist trained in BA. Support with the plan is part of how it was tested.
- If your depression is more severe, NICE advises weighing options with more therapist contact first [2].
- If you are grieving, a gentler week plan built on the same idea is on how to deal with grief.
When even small things feel impossible
Some days, washing one plate will feel out of reach. Make the step smaller still: stand at the sink, run the water. If that is all, it counts.
But if even the smallest activity feels impossible day after day for two weeks or more, that is a reason to see a doctor. The PHQ-9 depression test takes a few minutes and gives you words to bring to that visit.
Frequently asked questions
What is an example of behavioral activation?
Say you have stopped calling your sister and the dishes have piled up for a week. A plan might be: Monday, wash the dishes from one meal; Tuesday, send her a two-line message; Wednesday, ten minutes outside with a coffee. You rate your mood before and after each one, and at the end of the week you look at which ones helped.
Is behavioral activation CBT or DBT?
It grew out of cognitive behavioral therapy: it started as the "behavioral" part of CBT for depression and was later tested as a therapy of its own [7]. It is not part of DBT (dialectical behavior therapy), which was built for a different set of problems.
Does behavioral activation work for anxiety?
It was designed and mostly tested for depression. In the online trials pooled by Alber and colleagues, anxiety also went down compared with inactive control groups [6]. For anxiety on its own, other methods have been studied more; see how to stop overthinking and how to calm your nervous system.
How long does behavioral activation take to work?
NICE describes guided self-help as usually 6 to 8 sessions and individual behavioral activation as 12 to 16 [2]. Our advice: give a plan at least two or three weeks before you judge it.
What if I do the activities and feel nothing?
That is common in the first days, and a "0 to 0" rating still counts as data. Keep the activity if it matters to you, make it smaller, or swap the kind. If nothing at all shifts after a few weeks, talk to a doctor. It does not mean you did it wrong.
When to see a professional
Behavioral activation is a treatment method, and a self-help plan is the lightest form of it. Get medical help soon if:
- low mood or loss of interest has lasted most days for two weeks or more;
- you can't manage the smallest step on the plan, day after day;
- you sleep or eat much more or much less than usual, or can't work;
- you drink or use drugs to get through the day;
- you have thoughts of death or of hurting yourself. Then use the crisis numbers below today.
A doctor can check for physical causes and talk through therapy, medication or both.
